Yes it does, along with two speaker cutouts.

It's setup for an 8" free-air or infinite baffle subwoofer, and two 3.5" speakers. You would most definitely need an amp, how else is the thing going to get power?

Yes. Some amps accept high level inputs (speaker wire connections) and you just need to tap into the wiring for a left and a right channel speaker. You can do this at the door, or at the radio. If the amplifier you buy does not accept high level inputs, you will need a line output converter. This will take high level connections and convert them to low-level (RCA connections) that almost every amp supports.

      Everything is discussed HERE already

      plus Bonus

I want to sell my 12' sundown audio and mono Rockford amp and buy 8' free air and 2 set of 3.5s, two speakers for rear deck and 1 speaker as a central channel, plus amp, equalizer (stock is a JOKE) and somehow connect everything to stock head unit (I wonder what will happen with warranty). In future maybe I'll change 6x9s to 2 way 6.5s. Plus I need sound deadener for rear deck.

Can you guys post photos of stock Alpine audio, how it's installed. I mean subwoofer is under cover or ?

      xD lol

Not really difficult if you do it yourself. Expensive if you pay someone else to. the OEMs really dont' like you messing with the factory set-up.

I'm Thinking about Building a Stealth Box, For one 12" I don't like Free-Air subs. They are hard to find, and they aren't the best. I also plan on taking out the dyno mat in the cut out. Quick question Where do you plan on splicing the line output converter? I'v always Spliced it to speakers in the deck. The dart has none so where would be the best splice point; behind the Uconnect?

That looks like the easiest place to grab the speaker wires. If there is enough room, I'd just hide the Line Output Converter there and run the RCA's to the back like you replaced the deck with an aftermarket. You'll want to leave everything open so you can adjust the gain on the Line Out Converter before stashing it into the dash.
